How does the health system work for us long-term farangs in Thailand?

In the past, I have visited for six months at a time and had $1,000,000 holiday health cover.

I needed to use a hospital once so went to the private Bangkok Hospital, paid for the treatment, and then was repaid from the insurance company.

My travel insurance has now expired as I am living in Thailand now so the options are to buy health insurance in Thailand or pay cash when needed.

I looked at insurance and due to being in my 60's the monthly fees are high for low coverage.

I have put two million baht in an instant account in the meantime in case of emergencies.

So what rights do we have to access public hospitals, what are the costs, etc or would it be better for me to use a private hospital?

Can anyone recommend a medical insurance company?

What are your thoughts on medical insurance in Thailand or the use of the various types of hospitals?

Any information will help, thanks in advance.
