Beyond the undeniable stupidity of base jumping, what's interesting is that he had previously successfully done 500 jumps and he messed this one up by himself. The equipment wasn't faulty, he just didn't equip it properly and didn't check it before jumping. He was absent-minded that time, for some reason.
