She was under a weak heart beat and looks like her heart is pumping blood under huge stress. She should have been under constant monitoring at least 24 hrs as the possibility of a cardiac arrest is always there. Looks like the last few steps that she took after being discharged is the killer for her.

One does not feel pain in the heart. There are no  nerves for that. That is why when you have a heart attack you have pain elsewhere. So right off the start the son  is imagining some things. If the EKG was normal and the blood tests showed no enzymes indicative of a heart issue, then there wasn't much the hospital could do, especially with a patient who  had underlying conditions. Hearts do give out and people do die.
