What does the Vaccine Card look like?

For anyone that got vaccinated, what did the proof of vaccination card look like? Or did they give you anything else to prove your vaccination status??

You're probably asking about Thailand but Canada just emailed me a pdf document lol I was surprised.

I saw other countries have a physical card where their vaccine date/info was written on a card though (more like a small cardboard booklet sort of thing you get for other vaccinations)

I'm curious what Thailand went with
