How can a mentally unstable person become a police officer in the first place. Don't they have a regular health and mental stress check for such officers before they are allowed to continue their duties? It is always the same story when something happens....."mental health issue"

You don't have to be mentally unstable to become a police officer but it certainly helps.

You have assumed  he was unstable prior to his becoming a police officer. It is likely that the man developed his condition after some time as a  cop. He is a Lt., so he has been there 10+ years.

You have no idea as to what checks were done prior to this event. It is obvious, some action had been taken.  You are deluded if you believe that any police officer would want to work with  someone who was a danger to other police officers. Read the news item again: The man was already off duty because of mental health issues. What else  did you want the police to do? They can't just lock up people presumed to be  mentally ill. It requires family involvement and a court order.
