Sad to hear. I remember seeing airplane wreckage at Katmandu Airport in the early 90’s while landing and then leaving l. Was bazaar. May even have a picture of it in my archives some where. 
Also remember flying from Katmandu to Lukla. Quite the hard bumpy ride and quite the OMG landing into the short mountain strip. Nepal is worth it and the adventure. Well at least it was a long time ago. I wouldn’t bat an eye to go back and do it all over again with a friend or my son.RIP to all the deceased.

Neither of those things should lead to an accident.   There was a similar accident in the US some years ago, which was caused by incorrect use of the thrust levers and integrated prop controls, but this is of course pure speculation.
